COVID-19 notification eForm no longer required

The department's service provider COVID-19 eForm is no longer in use.

Community (including child and family services), disability and homelessness service providers can email if they require support to arrange testing or manage a confirmed or suspected COVID-19 case.  

Notifications from external service providers of Child Protection clients who are COVID-19 positive and queries relating to support for Child Protection clients, can be emailed to  

The internal Child Protection COVID-19 tracker remains in place for monitoring isolation and positive cases for clients and staff.
